Leave Application – Modal Window not Mobile usable

The Take Leave button loads a modal screen. This works fine with desktops/laptops. But on smartphone screen, it is kinda responsive, but very user unfriendly. In fact, it is not usable.

Is there a setting change to make this mobile-ready? Alternately, can this functionality be loaded to a page/post, instead of a modal window?
